Covers all active full-time and part-time benefit employees (YKHC-Payroll Paid) who are regularly scheduled to work an average of 20 hours per week, averaged over the lesser of:
A. The prior 12-month period; or
B. The period of employment with
the employer.
Generally, an employee will become eligible to enroll as a covered person on the 1st of the calendar month following his or her date of hire, or the date the employee becomes benefit eligible.

Eligibility and Entry Dates for YKHC 403(b) Retirement Plan – Employee Contributions
All current employees are eligible to participate in the plan. No age or service requirements apply for participants making deferrals to the 403(b) plan. Subject to IRS plan maximum limits.
Eligibility and Entry Dates for YKHC 401(a) Retirement Plan – Employer Contributions
Participation in the plan is open to employees who have met ALL of the following requirements:
Attainment of age 18, Completion of 1 year, 1,000 hours of service and employed on December 31 of the plan year.
Other requirements may also have to be met, as described in the Summary Plan Description.
You may participate in the plan on January 1, April 1, July 1, or October 1.
